Abstract
Statement of Problem: This project addresses the urgent issue of opioid use disorder (OUD) among formerly incarcerated Black men in Sacramento County, a population disproportionately impacted by substance use, stigma, and barriers to treatment and reentry services.
Analysis: Guided by the Social Ecological Model, the project analyzes multiple factors contributing to opioid overdose risk at individual, interpersonal, community, institutional, and societal levels. These factors include risk behaviors, unstable living conditions, limited access to healthcare, institutional inequities, and systemic racism.
Intervention (Solution) Proposal: The proposed intervention, Sac 2nd Chances, is a reentry support program designed to improve health outcomes and reduce recidivism by providing transportation assistance, peer-led education, and linkages to harm reduction services, medication-assisted treatment (MAT), healthcare, housing, and employment resources. The program aims to empower participants, reduce stigma, and increase engagement in evidence-based treatment and support services.
