Abstract
Problem Identification Hypertension, also commonly known as high blood pressure, is a condition characterized by increased levels of blood pressure. As a community, Sacramento County has a high prevalence of hypertension. In South Sacramento, specifically in the 95823 ZIP code, African American adults have been identified as a community of concern. Although hypertension affects many individuals, the condition may be reduced and easily managed by making certain lifestyle changes.
Analysis Literature suggests that sodium consumption and fruit and vegetable consumption are modifiable risk factors directly associated with the prevalence of hypertension. Using the Social-Ecological Model, the important individual and ecological risk factors were identified. Major concerns related to these risk factors are knowledge, self-efficacy, and access.
Intervention (Solution) Proposal The proposed intervention is to implement a 6-month nutrition education and access program at the South Sacramento Christian Center in 95823 targeting African American adults residing in this community. This intervention will provide the skills and knowledge to modify dietary intake and vouchers to purchase farmer's market produce.
Implementation and Evaluation Successful implementation of the intervention will require a coalition of many organizations. Despite the strengths of the intervention, further research and efforts may be needed to measure its success and ability to be effectively replicated in other communities.
