Abstract
Problem Significance: Hypertension is described as the most important modifiable risk factor for all-cause morbidity and mortality worldwide. If this condition is not controlled, it can lead to complications such as heart disease, stroke, or kidney disease. Sacramento County was found to be at an increased risk of developing hypertension and hypertension-related complications. If no action is taken, hypertension will continue to decrease quality of life, length of life, and burden the economy.
Analysis and Objectives: Reviewing the literature, physical activity and high sodium intake were recognized as important and modifiable risk factors for hypertension. Using the Precede-Proceed model, behavioral determinants, environmental determinants, and their PERFs were identified. Current efforts addressing the issue were discussed and found mobile app interventions to be successful. Project goals, outcomes, and objectives were then developed to guide the proposed intervention.
Intervention (Solution) Proposal: The intervention “Fitness for Hypertension” aims to decrease the prevalence of hypertension by increasing knowledge of hypertension and increasing the number of people who meet the recommended weekly amount of physical activity. The intervention has five main sections and is implemented in four phases. An evaluation plan is also provided to measure program objectives.
Recommendations and Conclusions: Recommendations include solidifying partnerships, following timelines, and ensuring cultural competency. Its strengths include being grounded in theory, addresses one of the most important factors in addressing hypertension, and is highly adaptable. Despite its strengths, limitations like limited data and adherence to the program exist.
