Abstract
Problem Identification
Type II diabetes is a chronic disease that affects nearly 40% of all American adults, and cases continue to rise on both the national and local scale. Hispanic adults in Sacramento County were chosen as our target population as they face a much higher disease burden than White adults and have a household income of nearly $18,000 less annually than White households.
Analysis
The Precede-Proceed model was used to identify key factors contributing to diabetes in Hispanic adults in Sacramento County. Diabetes control is affected by an individual’s genes, medication adherence, and availability of comprehensive diabetes care. Furthermore, an individual’s understanding of the risks of diabetes, family support, medication cost, and availability of healthcare clinics affect their medication adherence and ability to obtain comprehensive diabetes care.
Intervention
Diabetes Direct is a proposed intervention designed to target factors at multiple ecological levels that contribute to diabetes control in our target population. This program will apply the Transtheoretical Model to meet the priority population’s needs more appropriately, and participants will be recruited from the 95825 zip code in Sacramento County.
Implementation and Evaluation
Diabetes Direct will employ five staff members and partner with local hospitals, churches, and other community establishments for implementation and evaluation. A community advisory board will be created for process evaluation, and participants from the intervention and comparison groups will complete a self-report pretest and series of posttests.
