Abstract
A 63 year-old female patient with multiple sclerosis was seen for physical therapy treatment for 12 sessions from 02/13/2014 to 04/10/14 at an outpatient clinic under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with Multiple Sclerosis Impact Scale-29, 6 Minute Walk Test, Dynamic Gait Index, modified Clinical Test of Sensory Interaction on Balance, Fatigue Scale of Motor and Cognition Scale, and a plan of care was established. Main goals for the patient were to improve endurance, decrease fall risk, and improve balance. Main interventions used were rhythmic auditory stimulus, and vestibular training. The patient achieved goals of decreased fall risk and improved balance. The patient was discharged to home with home exercise program.
