Abstract
A patient with multiple sclerosis was seen for physical therapy treatment for five sessions from 4/10/14 to 5/13/14 at an outpatient physical therapy clinic under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with hand-held dynamometry, posture, diaphragm function, the Berg Balance Scale, the 2 minute walk test, Activities-Specific Balance Confidence scale, the Borg Rating of Perceived Exertion, the Modified Fatigue Impact Scale, and the Multiple Sclerosis Quality of Life Survey and a plan of care was established. Main goals for the patient were to increase tolerance for walking longer distances and improve quality of life. Main interventions used were task specific, bodyweight supported treadmill training, and core stability training. The patient achieved the following goals: improved dynamic balance, decreased exertion level when walking from bed to desk chair in home, improved diaphragm function, improve core stability and improved quality of life. Upon discharge, the patient continued living at home.
