Abstract
A 74-year-old female patient was seen for physical therapy treatment due to an ischemic stroke of the middle cerebral artery impacting the lenticulostriate branches, with infarction of the basal ganglia and corona radiata. She was seen for 4 sessions over 4 days by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with Manual Muscle Testing, Rancho Los Amigos Observational Gait Analysis, the Postural Assessment Scale for Stroke, the Tinetti Performance Oriented Mobility Assessment, the Functional Ambulation Category, the Activity Measure for Post-Acute Care mobility domain, and the Barthel Index. She also was given the line bisection test to rule out visuo-spatial hemi-inattention and the Orpington Prognostic Scale to help inform her prognosis and discharge destination. A plan of care was established to address impairments with strength, gait mechanics, balance, ambulation, functional mobility, and ability to perform activities of daily living. The patient had goals to return as much function to her right side as possible and regain independence with functional mobility. Main interventions used were repetitive, task specific strength, balance, gait, functional mobility, and activity of daily living training. The patient achieved the following goals: improved strength, improved static and dynamic balance, improved ambulation ability, and improved functional mobility. The patient was discharged to an inpatient rehabilitation facility.
